Adding 2nd RX 5700 error

I have a H110 BTC Pro motherboard. Running Hive OS I installed one ASRock RX 5700 and it worked fine. I went to install the second one and it ran for about 10 or 20 minutes, then there was an error. I tested the risers and cables and they were okay. What I did next was test the ASRock RX 5700 that didn’t work as a solo card. It starts up and runs, but then the screen immediately goes green. I am trying to figure out what this means and what I can do to fix it.

Here is the error immediately prior to turning green.

I see bad drive
So first solve this issue

Bad drive as in storage? I am running HiveOS latest beta from USB. for Driver, i thought they were updated from HiveOS image.

yes bad storage drive. See on your picture (io error on drive sr0 )

@HaloGenius Thank you. I was wondering what the error was for that. I reformatted the flash drive . I found out that a certain card did not like the OC setting, so when i reset to normal it works.
